[5.0.1] Strange Dialog Title Bar Appearance on 4k Monitors

We're using 4k monitors and noticed that the title bars for all our QDialog windows have the wrong size and layout.

This can be seen by using QDesigner and creating a new dialog from the wizard. The appearance looks like the attachment.

NOTE: this only occurs when the QtitanRibbonDesigner5.dll has been added to the "plugins/designer" folder for Qt. If we remove the DLL, then the QDialog appears with the correct title bar height and system-button position.

We had to build the 5.0.1 QtitanRibbonDesigner5.dll from scratch because we're using Qt 5.12.4. The QTitanRibbon 5.0.1 release has a DLL pre-built for Qt 5.13.0.

I'm installing Qt 5.13.0 to test that next.

Also confirmed same behavior in Qt 5.13.0 Designer with stock QtitanRibbonDesigner5.dll that ships with 5.0.1.

I'm using a 4k display with 150% DPI scaling.
